The child, known as Kiwi to his friends and family, was pronounced dead after being taken to hospital from an address in Old Moat Way in the Ward End area of Birmingham.
Shuttleworth, of Cottingham Road, Manchester, is further charged with causing or allowing Keanu's death and two counts of child cruelty.
Southerton, of Hagley Road, Edgbaston, Birmingham, faces further counts of causing or allowing the death of a child and one charge of child cruelty.
Both defendants spoke only to confirm their personal details during the preliminary hearing at Birmingham Crown Court.
